Vision Benefit for Four Year Olds

The Vision Benefit for Four Year Olds provides eyecare to four year old children who are about to start school in New Brunswick.

This program covers one major eye exam and, if required, one set of corrective glasses to ensure all children receive proper vision care before starting school.

The Benefit is an expansion of the Healthy Smiles, Clear Vision program. It is available for parents or guardians with or without vision insurance. It is administered by Medavie Blue Cross.

To be eligible for the Vision Benefit for Four Year Olds, your child must be:

  • Four years old
  • A resident of New Brunswick

Documents listed in this section are examples of what can be included with your application. Please review the application instructions (How to Apply tab) to confirm the supplemental and required documents for this benefit.

Proof of  residency in Province/ Territory

For example:

  • lease agreement
  • rent receipt
  • household bill
    • gas
    • electricity
    • cable television
    • telephone
  • driver's license
  • vehicle registration or car insurance
  • membership in social or professional organization
  • Other:
    • bank information
    • employment information

Proof of primary responsibility for the care and upbringing of the child

For example:

  • a letter from the day care or school authorities indicating the child’s home address and contact information on file
  • a letter from a social worker, physician, a band council, or a resettlement officer stating that they have personal knowledge that the child lived with you for the period you indicated
  • a registration form or a receipt from an activity or club the child was enrolled in for the period you indicated
  • a court order, decree, or separation agreement that explains the type of custody arrangement you have (for example, shared custody) and clearly shows the living arrangements for the child
  • any other document showing that the child lived with you for the period you indicated

Health card

For example:

  • valid health card with no photo, name and address
  • valid health card with photo, name and address
  • MCP (Medical Care Plan) number
  • Valid ambulance/dental services card
  • Department of Immigration, Skills and Labour identification number
  • Department of Immigration, Skills and Labour file number (Income Support Card).

To apply for this program, you must:

You will need to provide a copy of your child’s N.B. Medicare card and details on any health insurance coverage you may have with your application.

Please submit the application form at least ten business days before your child’s vision appointment to give Medavie Blue Cross time to process your application.

You will receive a benefit confirmation letter in the mail. Please bring this letter to your vision appointment. The identification number included with the letter is required for submitting claims to Medavie Blue Cross.
