To qualify for the Extended Health Benefits for Specified Disease Conditions Program:
- A permanent resident of the Northwest Territories (NWT)
- Métis or non-Indigenous
- Have a valid NWT Health Care Card
- For the Specified Disease Conditions Program, you must be under the care of a physician or nurse practitioner for one of the disease conditions listed on this page.
