To get this benefit you need to first apply for the following benefit(s) if you're eligible:

If you need to attend appointments for medical reasons, Ontario Works or Ontario Disability Support Program may cover these costs. This includes going to medical, rehabilitation, or counseling appointments.

To qualify for Transportation for Medical Treatment, you must have spent a minimum of $15.00 in approved medical transportation costs in any given month.

You may be eligible if you:

  • Are receiving Ontario Works or Ontario Disability Support Program Income Support.
  • And/or your family spend $15 or more a month on medical transportation.

Health card

For example:

  • valid health card with no photo, name and address
  • valid health card with photo, name and address
  • MCP (Medical Care Plan) number
  • Valid ambulance/dental services card
  • Department of Immigration, Skills and Labour identification number
  • Department of Immigration, Skills and Labour file number (Income Support Card).

Apply by contacting your Social Service Worker at Ontario Works or Ontario Disability Support Program to find out if you are eligible.
